I, the parent mentioned above, herewith certify that my child/children mentioned above is/are in good health condition, has/have no medical problems and is/are fit for physical activities in Singapore, especially considering the climate.

I hereby give consent for my child/children to take part in the activity & consent to any emergency treatment as necessary. I accept that Cosmo United Football Academy (CUFA) and their employees and coaches are not under liability whatsoever in respect of injury, loss or damage to my and/or my child’s/children’s health or property whilst participating in any coaching sessions, camps, tournament or other activities wherein my child/children or I participate. I understand that participating in any CUFA activity is at my child’s/children’s/mine own risk. CUFA, its owners, employees and coaches shall not be liable for any damage whatsoever, even in the event of negligence or gross negligence by CUFA, its owners, employees and coaches, arising from any personal injury, death or property loss sustained by participants and families during any coaching, camp, tournament or other activities related to the participants and families enrolment in CUFA.

CUFA treat as a priority the safety and well-being of all children attending our courses. CUFA, therefore, reserves the right to expel from any of our activities and without refund any participant or its family who is found bullying, misbehaving or who is generally disruptive.

I herewith consent that photos or videos may be taken of me/my children taking part in CUFA activities and that CUFA may use these photos or images for it marketing purposes. CUFA may especially (but not only) use the photos/videos on its website/social media presence or on flyers or other marketing material. CUFA may also share my contact details with business partners, who may contact you for marketing purposes. In addition, CUFA may share my contact details with other participants of CUFA activities. Please do inform us if you do not wish to consent to all aforementioned data processing activities.

Pro-rata payment is only possible for new sign-ups. Existing CUFA members need to make full term fee payment and may use make up classes to cover missed classes within the term.
